- CNIC
- Domicile
- Recent passport-size photographs
- Verified Degrees/Transcripts (Only HEC-attested accepted)
- For Professors/Associate Professors: Submit 4 DVDs with soft copies of documents (academic, experience, publications)

Attach the original bank challan with your form.
